WHO declares global health emergency over Ebola outbreak in Congo and Uganda

ABUJA – The World Health Organization (WHO) has officially declared the recent Ebola outbreak, caused by the rare Bundibugyo virus in the Democratic Republic of the Congo (DRC) and neighboring Uganda, a public health emergency of international concern. This announcement comes in light of alarming statistics: over 300 suspected cases and 88 reported deaths.

Despite the severity of the outbreak, the WHO clarified that it does not meet the criteria to be classified as a pandemic emergency like COVID-19 and advised against the closure of international borders. This nuanced stance aims to balance public health concerns with the need for continued global connectivity.

A laboratory-confirmed case has emerged in Kinshasa, the capital of Congo, which is approximately 1,000 kilometers (620 miles) from the epicenter of the outbreak in Ituri province. This development raises concerns about potential wider transmission of the virus, as the affected individual had traveled to Ituri. Additionally, suspected cases have been reported in North Kivu province, which borders Ituri and is one of Congo’s most populous areas.

Ebola is a highly contagious disease that spreads through bodily fluids, including vomit, blood, and semen. Although outbreaks are rare, the disease can be severe and often fatal. The WHO’s emergency declaration is intended to galvanize response efforts from donor agencies and governments, indicating the seriousness of the situation and the potential for international spread.

Historically, the global response to such declarations has been inconsistent. For example, when the WHO declared mpox outbreaks in Africa a global emergency in 2024, the rapid provision of supplies like diagnostic tests and vaccines was lacking, highlighting systemic issues in pandemic preparedness.

Understanding the Bundibugyo Virus

The current outbreak is attributed to the Bundibugyo virus, a rare variant of Ebola that has no approved treatments or vaccines. While Congo and Uganda have experienced numerous Ebola outbreaks, this marks only the third detection of the Bundibugyo variant. The WHO reports that Congo accounts for nearly all cases, with only two confirmed in Uganda.

The Bundibugyo virus was first identified during a 2007-2008 outbreak in Uganda’s Bundibugyo district, which resulted in 149 infections and 37 fatalities. The second documented outbreak occurred in 2012 in Isiro, Congo, with 57 cases and 29 deaths.

Challenges in Containment and Response

Dr. Jean Kaseya, Director-General of the Africa Centres for Disease Control and Prevention, highlighted the complications surrounding containment efforts due to a high number of active cases in the community, particularly in Mongwalu, where the outbreak was first reported. Ongoing violent conflicts, including militant activities linked to the Islamic State group, and continuous population movements for mining activities further complicate response operations.

The outbreak’s spread was first reported in Ituri province, close to the borders of Uganda and South Sudan. Recent updates from the Africa CDC indicate 336 suspected cases and 87 deaths in Congo, emphasizing the urgent need for effective containment strategies.

Dr. Tedros Adhanom Ghebreyesus, WHO Director-General, noted significant uncertainties regarding the true number of infections and the geographical spread of the outbreak, underscoring the need for intensified public health efforts.

The Importance of Rapid Response

Initial reports indicate that the outbreak began in April, with the first known case developing symptoms on April 24 and dying shortly thereafter. Alarmingly, health authorities were only alerted to the outbreak via social media on May 5, by which time 50 fatalities had already occurred.

The WHO has also reported that at least four healthcare workers have died after exhibiting symptoms of Ebola, highlighting the dangers faced by frontline responders.

Addressing the Gap in Treatment and Vaccination

The Africa CDC’s principal adviser, Shanelle Hall, noted that while four therapeutics are under consideration for treating the Bundibugyo virus, no vaccine is currently being developed. This lack of preparation for a rare virus poses significant challenges, especially given that existing Ebola vaccines are not produced in Africa. The COVID-19 pandemic underscored the necessity for African countries to boost their capacity for vaccine production, but resources remain limited.

Dr. Kaseya emphasized the need for Africa to develop its own manufacturing capabilities for medical supplies, stating, “If we are serious in this continent, we need to manufacture what we need. We cannot every single day look for others to come to tell us what they are doing.”
