Recently, the Trump administration announced a significant policy change regarding Medicaid coverage for gender-affirming care. Starting on October 13, Medicaid will no longer fund gender-affirming surgeries and hormone treatments for transgender minors, which includes individuals under 19 years old who are covered by the Children’s Health Insurance Program (CHIP).
This decision is part of a larger trend of policies targeting transgender individuals that began when Trump returned to office last year. While the specifics on how much taxpayer money is used for these treatments remain unclear, it’s important to note that at least 27 states have already prohibited gender-affirming care for minors. In many places, major hospitals have stopped offering these essential treatments altogether.
The new policy will still allow Medicaid and CHIP to cover mental health treatments for transgender minors but will phase out coverage for hormone treatments by April 2027. This includes treatments used to block puberty and facilitate the development of sex-related physical characteristics.
Trump emphasized his stance on social media, stating, “We are not going to pay for our innocent children to undergo these barbaric surgeries and practices, which result in unthinkable and irreversible harm to their young bodies.” However, many experts and advocates for LGBTQ+ youth disagree with this perspective.
Josh Rovenger, a legal director at GLBTQ Legal Advocates and Defenders, argued that this rule restricts parents’ ability to make informed medical decisions for their children and is likely to face legal challenges. The Human Rights Campaign (HRC) also criticized the policy, stating, “The Trump administration is terrorizing trans youth and their families with these kinds of actions, and it has to stop.”
Notably, the American Medical Association and other major medical organizations support access to gender-affirming care, highlighting its importance for the mental and physical well-being of transgender youth. While the American Society of Plastic Surgeons noted a lack of evidence supporting the benefits of certain surgeries for minors, the conversation around gender-affirming care continues to evolve.
In addition to cutting Medicaid and Medicare funding for hospitals providing gender-affirming care for minors, the administration has also implemented policies affecting transgender athletes and options for gender designation on passports. Legal challenges against many of these changes are ongoing, with varying outcomes in courts.
